As an affiliate of CoxHealth, you’ll have access to one of the region’s largest local provider networks, encompassing dozens of hospitals and clinics, staffed by outstanding physicians and specialists.

PHCS Out of Area Network

To keep you covered wherever you go, we are pleased to partner with PHCS to provide Preferred Provider Organization (PPO) network services outside of our own network area. When seeking care outside of the Cox HealthPlans service area below, PPO members can lower their out-of-pocket expenses by taking advantage of this wrap network.

PHCS is one of the leading PPO networks in the United States. Their network consist of providers and hospitals in urban, suburban, and rural areas throughout the United States.

*The PHCS Out of Area Network is not applicable to Exclusive Provider Organization (EPO) plans.

Our EPO Network

The Cox HealthPlans EPO network offers comprehensive care within the preferred CoxHealth network. When you or your family need medical care, you can choose from doctors, health professionals, and facilities operating in our network of participating health professionals. These plans pay no benefits for out-of-network expenses except for Emergency and/or Urgent Care service situations, as well as certain mental health services.
