We are pleased to offer a 2015 qualifying event that allows dependent children to enroll onto a child-only policy if parents elect to disenroll them during their group plan's Open Enrollment.

What do I need submit?

Submit an application with the first month's premium payment within 31 days of the dependent's last day of coverage.

What documentation is needed?

Letter from employer stating Open Enrollment period, names of terming dependents, last date of coverage, signed by administrator/owner on company letterhead Or Provide Employer Documentation Form
