Member Service is available Monday through Friday, 8:00am until 5:00 pm CST. We are available to assist you with eligibility, benefits questions, and claim inquiries. 

Here are some useful items to have available when calling in to Member Service:

  • Member’s Identification Number located on the ID card
  • Patient’s Name
  • Name of doctor, hospital, or provider
  • Date(s) of service
  • Claim number (if known)

Please note that you will only be able to obtain information on a dependent if you are the parent of the child and you are also a covered member on their plan. A signed HIPAA release form will guarantee that you have access to the information you need on family members.
